The Pokémon that has the most number of errors or changes has to be Pikachu. In all version of the Pikachu cards (Basic edition, Jungle edition, and Promo version), there have been major differences to some of the most basic things.
For starters, there have been three different flavor texts for the Pikachus.

Game Boy Pokédex.
In addition to that, there's a slight difference between the 1st and 3rd edition Pikachu's picture.
